UC Diabetes Prevention Program Banner

Our next cohort will be starting on July 11th at 5:15 p.m. and will be held via Zoom. Please note that access to a scale is a requirement of the program. The tentative schedule is listed below. To enroll in the June cohort, please fill out the intake form (attached below) and send it to Jessica Bulleri at jbulleri@ucsc.edu using Virtru. Please note that once enrolled in the program, attendance is strongly encouraged for each session.
