Waivers

Using waivers

  • Important information about the activity that will not fit in the description area of the waiver can be supplied to participants, parents or guardians in a separate document. That document should then be referenced in the description arear. The document should be given the person signing the waiver for review and a copy of the document should be retained with the waiver.
  • Waivers cannot be signed by minors. Activity participants under 18 must have a waiver signed by a parent or legal guardian.
  • Waivers should be retained for 5 years after the end of the activity OR 5 years after the 18th birthday of a minor participant – whichever is longer.
  • The campus unit sponsoring the activity is responsible for retaining the waiver. Go to Volunteer Waiver and Election of Workers’ Compensation Coverage for additional information about this waiver.
  • Waivers can be scanned and retained electronically. 
  • Waivers should be used as provided and not changed except for the fillable fields.
  • Do not translate or attempt to explain the legal meaning of the waivers. They are written in an easy to understand style that should be self-explanatory. If needed, participants can seek outside help to translate or explain the legal meaning of the waivers. 
  • Anyone who refuses to sign a waiver should not be allowed to participate in the activity.
