Exception Requests
BFB-BUS-63: Insurance Requirements and Certificates of Insurance establishes the required insurance provisions to be included in contracts between Suppliers and the University. Suppliers include independent contractors, consultants, vendors, or other service providers providing goods or services under a contract, purchase order or other written agreement with the University. Only Risk Services can make exceptions to the supplier insurance requirements.
The financial consequences of using an under-insured or uninsured supplier are often difficult to anticipate. Risk Services recommends that units choose insured suppliers that present acceptable evidence of insurance.
CampusConnexions
CampusConnexions can help some suppliers comply with the university’s insurance requirements. Through the CampusConnexions vendor insurance program, the cost of general liability and professional liability combined is $750 per year, for UC work only. Cost and insurance terms and conditions are subject to change. The supplier is responsible for working directly with CampusConnexions.
Units should consider the availability of insurance through CampusConnexions prior to asking for an exception to the university’s system-wide insurance requirements.
As a consequence of using an uninsured supplier or an underinsured supplier, the campus unit making the purchase might become financially responsible for losses that would have been funded by a supplier’s insurance. For example, it is unlikely that there would be any funding available through the university’s own programs of insurance to reimburse a unit for expenses related to a supplier’s errors or omissions.
Insurance Exception Request
The Insurance Exception Request Form can be used by campus units to submit an insurance exception request when there is a compelling reason to use an under-insured or uninsured supplier.

What are the Cyber Liability insurance requirements?
The Cyber Liability insurance requirements vary by Protection Level Classifications and number of records.
- P1 and P2, if Appendix DS is not incorporated into the purchase order or other contracting documents, then cyber liability insurance is not required.
- P3 and P4, less than 70,000 records – this insurance policy must have minimum limits of $5,000,000 each occurrence and $5,000,000 in the aggregate.
- P3 and P4, 70,000 or more records – this insurance policy must have minimum limits of $10,000,000 each occurrence and $10,000,000 in the aggregate.
Is Workers’ Compensation required?
- Workers’ Compensation is required as required by applicable state law. Some suppliers might not be required by applicable state law to carry Workers’ Compensation; otherwise, Workers’ Compensation is required.
Is Auto Liability insurance required?
- Auto liability is only required when the supplier’s vehicles will be used for purposes other than commute purposes.
